Case Manager-Outpatient

Case Manager-Outpatient position provides direct service to individuals served, family members, and designated others, utilizing a Person-Centered process and Family Centered Practice Concepts. Coordinates, consults, and provides liaison work with physical health care providers and other agencies.

Requirements

• Bachelor's Degree in Psychology, Social Work, Special Education, Counseling, or other mental health-related field
• Michigan Department of Health and Human Services (MDHHS) Prepaid Inpatient Health Plan (PIHP)/Community Mental Health Services Programs (CMHSP) Qualifications as a Qualified Mental Health Professional (QMHP)
• Possession of required state licensure and ability to meet privileging and credentialing guidelines
• Valid Michigan driver's license and operable, insured automobile for authorized travel
