Job Description
Provides a medically prescribed physical therapy plan of care under the supervision of a Physical Therapist. Requires a graduate of a Physical Therapist Assistant Program and current Oklahoma licensure.
Requirements
- Graduate of a Physical Therapist Assistant Program accredited by the Commission on Accreditation of Physical Therapy Education.
- Current Oklahoma Physical Therapist Assistant licensure and Form 5 Supervisor Confirmation required.
- Basic Life Support (BLS) certification for Healthcare Providers.
