Nurse Case Manager- Adult ED

The Nurse Case Manager (NCM) reports to the Manager of Case Management and Care Transitions, coordinating a plan of care to achieve clinical outcomes and promotes a safe and timely discharge. This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system.

Requirements

• Bachelor's degree in Nursing from an accredited school of nursing and three years work experience as a Registered Nurse required.
• Prior leadership experience preferred.
• One year of Case Management experience preferred.
• Case Management certification preferred.
• Licensure as a registered nurse by the state of South Carolina or compact state required.
• Certification in Case Management preferred.
• Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required
