Requirements
- Graduate of an accredited United States School of Medicine; MD or DO
- Board Certified / Board Eligible in Family Medicine or Internal Medicine
- Current license in the state of practice or willing to become licensed in other states
- Current DEA Certificate
- ACLM Certification preferred
- BLS - Basic Life Support Certification
