Job Description
The Licensed Practical Nurse (LPN) provides direct oversight and support to the care team on-site through hands-on education, coaching, and modeling. This position ensures the health and well-being of participants through advocacy with medical providers, coordination with visiting nursing services, and participation in discharge planning following hospital or rehab stays.
Requirements
- Valid driver's license, reliable vehicle, sufficient auto insurance, and acceptable driving record.
- Must be a Licensed Practical Nurse (LPN) with an active Massachusetts license.
- Strong ability to work independently while prioritizing multiple tasks efficiently.
- Proficient written, verbal, and organizational skills.
- Adaptability to meet the changing needs of program participants.
- Demonstrated compassion, patience, and strong interpersonal skills.
- Must be able to obtain and maintain certifications in MAP, CPR, and First Aid.
- Must be at least 18 years old.
