Job Description
This position supports and assists families as they identify and meet their own goals through a family-centered case management model. The position also assists with the completion of child health requirements and other required screenings and assessments.
Requirements
- AA or higher in Social Work or Human Services or related human services field
- Bilingual (English/Spanish or English and other languages present in the local area)
- Reliable transportation
- Minimum automobile coverage required by state of employment
