CCBHC Triage Coordinator

Metropolitan Family Services is seeking a CCBHC Triage Coordinator to join their team. The role will involve conducting initial screenings for mental health and substance use disorders, gathering information for the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS), and providing referrals to primary care, housing, and other services.
