Job Description
We are seeking a Care Manager who lives in Idaho to support Idaho Medicaid. The role will involve managing a case load and working with the geriatric population. Excellent computer skills and diligence are required to multitask between systems, talk with members on the phone, and enter accurate contact notes. This is a fast-paced position with field work involved, including travel to surrounding areas.
Requirements
- 2+ years of health care experience
- 1+ year of experience working with persons with disabilities/chronic conditions long-term services and supports (LTSS)
- 1+ year of experience in care management
- Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
- Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance
