Social Worker Case Manager - Per Diem

Requirements

• Develops and coordinates inpatient discharge plans for patients who are considered at risk for adverse outcomes
• Assesses all inpatients to determine the need for post-discharge services
• Collaborates with the interdisciplinary team and patient and family to develop a plan
• Documents all discharge planning activities in the medical record
• Assists in identifying cost savings for treatment consistent with payor requirements and patient financial resources
