Job Description
Monogram Health is seeking a Nurse Practitioner to join their team in Westwood, NJ. The Nurse Practitioner will provide personalized medical care to patients with chronic kidney disease and/or end-stage renal disease/end-stage kidney disease. The role requires the ability to work autonomously and make assessments, develop treatment plans, and implement care plans consistent with the patient's plan of care.
Requirements
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
- Board certified for appropriate licensure
- Current and unrestricted DEA certificate
- Ability to work without direct supervision and practice autonomously
- 2 or more years of direct patient care required
- Managed Care/IPA/Health Plan experience
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home
