Job Description
Mountain View Hospital is looking for a Social Worker Discharge Planner to join our team. Assists patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care.
Requirements
- Licensed Social Worker
- Bachelor's degree (B.A.) in social service from four-year college or university
- 3+ years of clinical social work experience
- BLS Certification
