Individualized Home Support Professional - Oakdale, MN - Part Time

The Individualized Home Support Professional will work with adults with disabilities in their homes, providing support with daily living skills, community participation, health, safety, and wellness, and household management. Responsibilities include maintaining records and documentation, following medication instructions, and transporting individuals as needed.

Requirements

• accurately read and implement Coordinated Service and Support Plan/Addendums
• maintain up-to-date records and documentation for each person on case load
• relate well with people with disabilities
• be qualified for employment in a 245d licensed program as determined by the MN Department of Human Services
