Job Description
An OhioRise Moderate Care Coordinator is a professional working under Ohio Medicaid's OhioRise program serving children with complex needs. The role involves delivering community based, wraparound care coordination and guiding children and families through the OhioRise program.
Requirements
- Licensure as a practitioner in accordance with rule 5160-27-01 of the Administrative Code
- Completion of high-fidelity wraparound training program
- Experience providing community-based services to children and youth and their families or caregivers
- Valid driver's license
- Reliable personal transportation
- Good driving record
- Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability
