Job Description
We are seeking a compassionate, team-oriented nurse to join our Acute Care for Elders (ACE) Unit at Methodist Hospital. The ideal candidate will have a willingness to learn and provide Age-Friendly care to patients 65+ years old. Requirements
- Provide appropriate care specific to the age of the customer/patient
- Perform an initial and ongoing assessment to determine the assigned patient's ongoing nursing care needs
- Develop/review/revise and individualize the plan of care to address the patient's nursing care needs
- Implement interventions that are consistent with the established plan of care
- Evaluate patient care to determine response to care/interventions for optimal patient outcome
