The Home Visitor supports high-risk pregnant and postpartum individuals through outreach, home visits, education, and referrals while managing a caseload of 35–40 clients.
Requirements
- High school diploma/GED
- Community Health Worker certification (or obtained within one year)
- Valid Ohio driver’s license, auto insurance, reliable transportation, and willingness to travel throughout Lucas County
- Culturally sensitive with a pleasant, mature, and professional demeanor
- Strong organizational, communication (verbal/written), listening, and interpersonal skills
- Ability to follow instructions, use sound judgment, and work independently or as part of a team
- Patient, understanding, and able to handle challenging clinical situations
- Basic computer skills (Microsoft Windows, Word)
- Maintains confidentiality and presents a neat, professional appearance
