Home Visitor

The Home Visitor supports high-risk pregnant and postpartum individuals through outreach, home visits, education, and referrals while managing a caseload of 35–40 clients.

Requirements

• High school diploma/GED
• Community Health Worker certification (or obtained within one year)
• Valid Ohio driver’s license, auto insurance, reliable transportation, and willingness to travel throughout Lucas County
• Culturally sensitive with a pleasant, mature, and professional demeanor
• Strong organizational, communication (verbal/written), listening, and interpersonal skills
• Ability to follow instructions, use sound judgment, and work independently or as part of a team
• Patient, understanding, and able to handle challenging clinical situations
• Basic computer skills (Microsoft Windows, Word)
• Maintains confidentiality and presents a neat, professional appearance
