Job Description
Community health worker responsible for working effectively with Enhanced Care Management (ECM) members to improve their whole health through outreach and engagement activities. This position works collaboratively with members and families, and other professionals to navigate the healthcare system, overcome barriers to care, and promote collaboration between patients and their medical/behavioral care team.
Requirements
- High school/GED
- Two years of healthcare, behavioral health, or social services industry experience
- Valid CA Driver’s License and proof of auto insurance
- Bilingual (English/Spanish) preferred
