Job Description
Community Health Worker responsible for working with Enhanced Care Management members to improve their whole health through outreach and engagement activities. Collaborates with the designated primary care provider (PCP) care team and other professionals to achieve improved care and outcomes.
Requirements
- High school/GED
- Two years of healthcare, behavioral health, or social services industry experience
- Valid CA Driver's License and proof of auto insurance
- Bilingual (English/Spanish) preferred
