Job Description
The Support Coordinator provides case management and support coordination for individuals with Intellectual Disabilities (ID/DD) diagnosed individuals. They work with providers to ensure individuals have access to necessary services and support to optimize their community living. Starting salary is $39,000, with potential for increase based on experience.
Requirements
- Registered as a QDDP
- Maintains QDDP status and all required trainings/CEUs
- High School Diploma required
- Minimum one year of experience working with individuals with mental illness, substance use disorders, and/or intellectual/developmental disabilities
