Care Coordinator

The Care Coordinator is the primary point of contact and liaison for internal/external resident care and services. They provide case management/care coordination and advocacy by carrying a caseload and assisting clients in negotiating social services, providing follow-up and monitoring support.

Requirements

• High School Diploma or GED + 12 credits in
• A minimum of two (2) years' experience in a human service field with direct contact with the individual receiving services
• Maintain a valid Driver's License and acceptable driving record
