Job Description
New York Psychotherapy and Counseling Center (NYPCC) is seeking a Care Coordinator to join their team. The successful candidate will be responsible for managing a caseload of Health Home Care clients, developing Comprehensive Care Plans, and providing supportive services to enhance clients' quality of life.
Requirements
- Bilingual (English/Spanish)
- Experience with RMA required
- Experience with HARP clients preferred
- Knowledge of various resources and services within a community
- Excellent verbal and written communication skills
