Job Description
We are seeking a Case Manager to provide quality case management and discharge planning services to patients and their families. The ideal candidate will have a Bachelor's degree in Psychology, Social Work, or a related field, and one year of behavioral health experience.
Requirements
- Bachelor's degree in Psychology, Social Work, or closely related field
- One year of behavioral health experience
- Current Cardiopulmonary Resuscitation (CPR) certification
- Verbal de-escalation and physical intervention program
- Familiarity with continuum of care and clinical terminology
