Drug & Alcohol Case Manager

The Drug and Alcohol Case Manager provides case management services to individuals experiencing substance use disorders (SUD) and assesses client needs, develops individualized service plans, coordinates treatment, and connects individuals with resources such as rehabilitation programs, housing, and employment assistance.

Requirements

• Conduct initial intake assessments and determine clients' level of care needs.
• Develop individualized service plans (ISPs) based on client needs and goals.
• Work closely with the local SCA and navigate the PA WITS program
• Monitor client progress in treatment and adjust plans accordingly.
• Coordinate referrals to rehabilitation centers, outpatient programs, support groups, housing services, and other community resources.
• Work closely with treatment providers, social workers, probation officers, and other stakeholders to ensure continuity of care.
• Maintain accurate and confidential records in compliance with state and federal regulations.
• Educate clients and families on substance abuse, treatment options, and recovery resources.
• Assist clients with securing employment, housing, and other social services.
• Have the ability to input/coordinated authorizations for insurances, SCA and treatment court.
• Advocate for client needs within the legal and healthcare system.
• Participate in staff meetings, case reviews, and professional development opportunities.
