Requirements
- Complete an initial assessment of patient and family to determine home care needs.
- Regularly re-evaluate patient nursing needs.
- Initiate and revise care plans based on patient status.
- Administer medications and treatments as prescribed.
- Provide direct patient care.
- Counsel patient and family.
- Develop discharge planning plans.
- Coordinate patient care for assigned caseload.
- Active member of Arizona Nurse Practice Act.
