AVP Care Coordination

The System Care Coordination Leader will lead a team focused on optimizing patient care and resource utilization in Nuvance Health, supporting clinical teams, and improving reimbursement through effective utilization review, denials management, and discharge planning. This role requires strategic leadership, collaboration, and a data-driven approach to enhance patient outcomes and organizational efficiency. The leader will be responsible for establishing committees and promoting a culture of patient-centered care.

Requirements

• Bachelor's degree in nursing (BSN)
• Master's degree in nursing, health administration, or a related field preferred
• Current licensure as a registered nurse (RN)
• Minimum of 5 years of clinical experience in an acute care setting
• Minimum of 5 years of progressive leadership experience in case management or utilization review
• Proven leadership experience with a track record of success
• Strong knowledge of healthcare regulations, accreditation standards, and payer requirements
• Excellent analytical, communication, interpersonal, problem-solving, and decision-making skills
• Experience with data analysis and performance improvement methodologies
• Commitment to patient-centered care and interdisciplinary collaboration
• Familiarity with Windows applications
• CT or NYS RN License
• Working conditions: Moderate physical effort
