Job Description
Clinical Pharmacist - Digital Medicine - Remote. Monitor and evaluate drug therapy for appropriateness based on patient’s health status, institutional criteria/guidelines, and cost efficient measures. Manage pharmacy services to assigned areas for improving patient outcomes. Provide drug information to healthcare personnel. Participate with medical team on patient care rounds. Precept pharmacy students and residents. Provide in-services/education to pharmacy and other healthcare providers.
Requirements
- Graduate of an accredited School/College of Pharmacy
- PGY1 Residency in Pharmacy Practice
- Licensed to practice pharmacy in the State of Louisiana, or eligible for licensure by examination or reciprocity from another state
- Computer skills and dexterity required for data entry and retrieval of information
- Effective verbal and written communication skills
- Proficient with Windows-style applications, various software packages specific to role and keyboard
- Strong interpersonal skills
- Ability to exhibit good clinical experience in specialty as well as leadership/teaching skills
- Ability to understand the distributive process of the Pharmacy within 45 days from hire
- Ability to travel to offsite locations as required
- Ability to work flexible schedule as needed such as nights, evenings, weekends, etc.
