Family Nurse Practitioner, Community Health Center, Part Time- Cape Cod

Community Health Center is seeking an experienced Family Nurse Practitioner for a Part Time (20 - 28 hours) Benefited Outpatient Position on Cape Cod. The Nurse Practitioner provides comprehensive outpatient care, sharing responsibilities and collaborating with the physician in the care of patients.

Requirements

• Assessing, diagnosing, prescribing, treating, and patient/family education
• Diagnose and Treat acute and chronic Illness
• Perform age-appropriate history and physical exams
• Order and interpret diagnostic and therapeutic tests relative to patient’s age-specific needs
• Prescribe appropriate pharmacologic and non-pharmacologic treatments to patient’s age specific need
• Monitor the effectiveness of treatments
• Provide education of acute and chronic medical conditions
• Manage Chronic Medical Conditions
• Facilitate appointments to specialist
• Facilitate appropriate chronic care management (i.e. Diabetic eye exams, foot exams and nutritional counseling) and Preventive Medicine
