Nurse Practitioner - Post-Acute Care, Nursing Home

We are seeking a full-time Adult, Geriatric or Family Nurse Practitioner for our post-acute care locations in the north Boston area. The role involves delivering patient care to those in the community, working as part of a team in a collaborative care model, and enjoying a flexible schedule.

Requirements

• Post-Acute experience in a SNF/LTC setting
• Minimum 2 years as an NP in an acute or post-acute care setting
• Ability to work independently and manage complex clinical situations
