Clinical Care Transition Educator / Registered Nurse

Evaluate, educate, and train patients, caregivers, and facility staff about Option Care Health services and products to ensure successful transition of patients to an Option Care Health delivery model. Conduct patient assessments and evaluations to determine patient viability to join the OCH care delivery model. Coordinate the transition of patients from hospital to home or alternate care settings by collaborating with facility staff, providers, and internal teams.

Requirements

• Active and unrestricted Registered Nurse (RN) license
• Minimum of 2 years of experience in the healthcare industry
• Basic PC skills: internet, email, Microsoft Word, Microsoft Excel, Microsoft PowerPoint
