Requirements

• M.D. or D.O. degree
• Board certification or eligibility in an appropriate specialty
• Current medical license in Massachusetts
• Cardiopulmonary Resuscitation (CPR) certification and Advanced Cardiac Life Support (ACLS) certification
• Valid and unlimited license to practice medicine in the Commonwealth of Massachusetts
• Valid federal Drug Enforcement Administration number
• Other required state public health certifications with respect to prescribing privileges
