Requirements

• Reside in San Joaquin County
• 3-5 years in case management, social services, or healthcare
• Familiarity with Medi-Cal, CalAIM, and Enhanced Care Management
• Healthcare Insight: Understanding of healthcare systems and local community resources
• Strong communication, empathy, and cultural competence
• Proven time management skills and attention to detail
• Competence using case management software
• ValidCalifornia Driver’s License and personal vehicle
• Current liability insurance
• Pass a Motor Vehicle Report background check
