The Lead Care Manager is a direct service provider for individuals referred through the Managed Care plan for Enhanced Care Management.
Requirements
- Master’s degree (M.A./M.S.) in psychology, social work, public health or related field with at least 1 year of experience working with children and/or families.
- Or Bachelor’s degree (B.A.) in social services or related field, with at least 2 years of experience working with children and/or families.
- Other than BA degree with 2 years direct service experience working in case management or related experience in the field.
- Proficient knowledge of: Electronic Health Record, Microsoft Word, Outlook and Excel.
- Bilingual Spanish Required (verbal and written skillset).
- Valid California driver’s license; current automobile insurance; have and maintain a clean driving record acceptable to the organization’s insurance company; have immediate access to his or her vehicle during work hours.
