Requirements

• Residency in San Joaquin County
• 3-5 years in case management, social services, or healthcare experience
• Expertise in Medi-Cal, CalAIM, and Enhanced Care Management
• Understanding of healthcare systems and local community resources
• Strong communication, empathy, and cultural competence
• Proven time management and organizational skills
• Competence with case management software and related tools
• Valid California Driver’s License & Personal Vehicle
