Medical Social Worker

This Medical Social Worker position provides social work services to individuals and families in crisis, conducting assessments, developing treatment and discharge plans, and coordinating resources. The role involves collecting data, maintaining quality of practice, and fostering relationships with patients and external stakeholders. The ideal candidate will assess needs, conduct behavioral health assessments, and ensure access to social work services throughout the healthcare continuum.

• Conduct behavioral health assessments.
• Develop and coordinate admission, transfers, and discharge plans.
• Follow legal guidelines in mandatory reporting.
• Collect data and maintain quality of practice.
• Maintain working knowledge of community resources and assist in obtaining needed resources.
• Participate as a team member of the clinical interdisciplinary team.
• Maintain working knowledge of standard office equipment and computer skills.
• Fluent in English.
