RN Case Manager

We are hiring a RN Case Manager to provide support to patients and families in addressing medical and social concerns. The ideal candidate will have a current unrestricted RN license in Minnesota, at least 3 years of clinical practice experience, and demonstrated effective nursing judgment and communication skills.

Requirements

• Registered Nurse with current unrestricted license in the State of Minnesota, BSN preferred
• Minimum of 3 years clinical practice experience in an acute care setting
• Minimum of 3 years relevant utilization review, discharge planning, or case management experience
• Current clinical knowledge
• Effective, independent nursing judgment and skills
• Skill and experience in effectively collaborating with care team members
• Working knowledge of quality improvement, utilization management, benefit plans, fiscal management, and various payment methodologies
• Flexibility, organization, and appropriate decision-making under challenging situations
• Basic computer skills
