Job Description
We are seeking a Case Manager to join our Interim Housing Services team at the Yale Navigation Center in Santa Ana, CA. As a Case Manager, you will be responsible for providing supports to clients through all phases of homelessness, including intake and individualized needs assessment, referrals, linkages, and support to resources that help clients achieve their goals.
