Job Description
PATH (People Assisting The Homeless) is seeking a Mission and Values-Driven MSW Clinical Case Manager to support individuals experiencing homelessness in San Diego County. This role involves providing field-based, direct client outreach, resource navigation, and case management services to promote recovery and community integration. The position is integral to PATH’s comprehensive continuum of services focused on addressing the causes of homelessness and helping individuals achieve stability.
