YCS CLIENT SUPPORT PROVIDER

Pawnee Mental Health is seeking a motivated and passionate individual to provide support to clients with mental health diagnoses. The ideal candidate will have strong communication skills and be able to work collaboratively with clients and their treatment teams to achieve goals.

Requirements

• Review client person centered plans and develop activities to enhance functioning levels in identified areas of need
• Provide direct services in location most appropriate based on client need and document progress in each person’s chart
• Collaborate with family members and other members of the treatment team in identifying goals and strategies which reinforce desired behavior
• Advocate for client rights and community integration
• Attend and actively participate in clinical supervision and program staffing
• Meet client care hour expectations as outlined in agency policy
