Job Description
Peterson Health is seeking a Home Care RN Case Manager to coordinate and direct the care of patients in their home environment. The successful candidate will have a background in nursing and experience in healthcare settings, with a focus on case management and patient care.
Requirements
- Graduate of an accredited Diploma of Nursing Program or Associate’s Degree in Nursing
- Current Texas state license in appropriate discipline
- Current American Heart Association CPR
- Current Texas state driver’s license
- Current auto insurance
- One year experience in a healthcare setting in respective discipline
