Requirements

• Provide orientation to new patients
• Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans
• Administer biopsychosocial assessment
• Develop and implement individualized Treatment Plans
• Monitor patients' progress and document any changes
• Comply with Medical Q.A and Clinical Risk Management Policies
• Refer high-risk patients to Medical Directors, Physician Extenders, or Regional Clinical Leadership
• Provide professional counseling and referral services
• Engage in Community Relations and Outreach efforts
• Attend training and meetings as required
