Job Description
The Independent Contractor (IC) vasectomy provider will provide vasectomy-related services to PPIL patients, including the vasectomy procedure, pre-operative visit, and physical exam.
Requirements
- Provide vasectomy care, including post-procedure care, in accordance with PPIL's Medical Standards and Guidelines (MS&Gs)
- Previous work experience/training in the provision of vasectomy care
- Current physician license in the State of Illinois
