Job Description
Porter Cares is expanding into Michigan and seeking Nurse Practitioners to provide in-home health risk assessments. You'll evaluate patient history, conduct physical exams, perform point-of-care testing, and contribute to care management, focusing on a personalized and transparent healthcare approach. Travel will be required between Saginaw, Bay City, and Flint, Michigan.
Requirements
- Valid unencumbered Michigan NP license
- 3 years of patient care experience required
- Experience in primary care/adult/geriatric preferred
- EMR skills preferred
- Strong sense of empathy, compassion, and friendliness
- Exceptional communication skills, both written and oral
