Job Description
As a Mental Health Practitioner, you will provide respectful support and assistance to individuals in their homes and communities, helping them maximize their self-help skills and follow their individual program plan.
Requirements
- Prepare and facilitate groups and group discussion.
- Monitor, report and document any and all unusual behavior and overall medical condition of person served.
- Communicate concerns, suggestions and information to supervisors in a complete, accurate, prompt, respectful and timely manner.
- Report neglect and/or abuse of person served.
