MD/DO, 2 years of primary care experience, active state license, DEA, and AHA Basic Cardiac Life Support certification

Associate degree or equivalent experience required, 3–5 years of related experience, and proficiency in records management and payroll
Licensed Practical Nurse - Primary Care
High School Diploma/GED, Graduate of an accredited certificate program, 2 years of experience in clinical operations
Certified Medical Assistant - Primary Care
High School Diploma/GED, 2 years of experience, Basic Life Support certification
Licensed Clinical Social Worker (LCSW) - Primary Care
Master's degree in Social Work (MSW) with 2+ years post-master independent counseling experience
Family Medicine Medical Director - 0.2 FTE
Doctor of Medicine or Osteopathic Medicine Degree (MD/DO), 2 years of clinical medical practice in primary care preferred, Active state license and DEA in state of practice
Phlebotomist - Certified (CPT)
High school diploma, phlebotomy certification, and 2 years of clinical experience
Part-Time Primary Care Nurse Practitioner
Graduate of an accredited school of nursing and MSN required, with 1 year of clinical medical practice preferred
Medical Director - Primary Care
Doctor of Medicine or Osteopathic Medicine Degree, Board-certified in family practice or internal medicine, 2 years of clinical medical practice in primary care
Certified Medical Assistant - Primary Care
High school degree, 2 years of experience, and certifications required
