1 year of clinical medical practice preferred, MSN required, board-certified in family practice or internal medicine
Medical Director - Primary Care
Doctor of Medicine or Osteopathic Medicine Degree, 2 years of clinical medical practice, and active state license required
Physician - Primary Care
2+ years of clinical medical practice experience and a Doctor of Medicine or Osteopathic Medicine Degree (MD/DO) required
Registered Nurse - Primary Care
1 year of experience in Primary Care and/or Chronic Disease Management, active and unrestricted Nursing License, and strong computer skills, including EMR experience
Registered Nurse - Primary Care - FLOAT
1 year of experience, current and active state Nursing License, and strong computer skills
Registered Nurse - Primary Care - FLOAT
1 year of Primary Care and/or Chronic Disease Management experience, Current, active, and unrestricted state Nursing License, Current BLS for providers from the American Heart Association (to be renewed annually)
