Requirements

• Supervisory Visits and Education in a Patient's Home
• Re-evaluation of patients' skilled nursing/PSS needs
• Work autonomously with minimal assistance by the Clinical Manager
• Maintain patient confidentiality
• Make decisions based on established policies and procedures
• Preparation of the clinical progress notes
• Informing the family and Registered Nurse of changes in the patients' condition or needs
• Administration of medications and treatments as prescribed by a physician in accordance with currently accepted standards of medical practice
• Total patient care
• Patient and family teaching
• Reports directly to the Registered Nurse and Clinical Director
