Case Manager

Case Manager will provide assessment, case management and short-term counseling to clients to achieve outcomes of maintaining housing, living productively in the community, managing mental illness, substance abuse recovery, stable physical health and managing finances and entitlements independently.

Requirements

• Bachelor's degree in social work, psychology or other human services field from an accredited college or university
• Minimum of one year of experience working with homeless individuals with either a mental illness, substance addiction or MICA diagnosis
• Knowledge of case management software and Microsoft Office Suite
• Bilingual English/Spanish a plus
