Case Manager

The Case Manager is responsible for providing assessment, case management, and short-term counseling to clients to achieve outcomes such as maintaining housing, managing mental illness, and managing finances independently.

Requirements

• Bachelor's degree in social work, psychology, or other human services field
• Minimum of one year of experience working with homeless individuals with mental illness, substance addiction, or MICA diagnosis
• Knowledge of case management software and Microsoft Office Suite
• Bilingual English/Spanish a plus
