Job Description
Director of Care Management RN will execute strategic vision, lead care management functions, and ensure seamless care transitions. The role involves formulating and executing annual plans, directing care management, and promoting a culture of learning and excellence.
Requirements
- Bachelor's Degree in Nursing required; Master's Degree in Nursing or related healthcare field from an accredited program (Preferred)
- 5 years of relevant experience in an acute healthcare setting in care management, coupled with 3 years of progressive leadership experience
- California Registered Nurse License upon hire
