Home Visit Reablement Physiotherapists Required in London

We are seeking a dedicated and experienced Physiotherapist to join our client's team in South-West London, providing short-term home-based rehabilitation for patients recently discharged from hospital.

Requirements

• HCPC registration (Health and Care Professions Council)
• Degree in Physiotherapy
• Experience in community rehabilitation, home-based assessment or reablement
• Strong clinical reasoning and communication skills
• Ability to work independently and as part of a multidisciplinary team
