Medical Social Worker (MSW) to join our San Ramon-based home health team. Provide psychosocial support, counseling, and care coordination for homebound patients.

Requirements

• Master’s Degree in Social Work (MSW) from an accredited program
• LCSW preferred, but not required
• Minimum 1 year of clinical experience (home health or hospice preferred)
• Current CPR certification
• Valid California driver’s license, reliable transportation, and active auto insurance
• Strong communication, advocacy, and time management skills
