Job Description
The Case Manager provides comprehensive case management services to individuals with serious and persistent psychiatric disorders, or co-occurring disorders, and social service needs. The position involves coordinating admissions, arranging transportation, and developing discharge plans to support successful community reintegration.
Requirements
- Associate/bachelor's degree in social work, psychology, counseling, or a related field
- Minimum of 2 years of experience in case management or related social services field
- Knowledge of case management principles and practices
